Optional dental insurance

Summary

This optional insurance plan is available to you and your family.

It offers:

  • 50% reimbursement for major services, up to an annual maximum of $1,500 per person covered
  • 50% reimbursement for orthodontics, up to a lifetime maximum of $2,500 per person covered

Premiums are paid entirely by you.

Eligibility

You must be covered under the Basic Dental Insurance Plan to be eligible for coverage under the optional plan.

You are eligible if you are:
  • A term employee who has completed 1 year of continuous service
  • A permanent employee
  • under age 71

Individual or family coverage will automatically match the same coverage as your basic dental plan.

Premiums

You pay the following monthly premiums:
  • Individual coverage: $9.97

  • Family coverage: $33.79

Services

Reimbursement is based on the Ontario Dental Association Fee Guide for the year immediately preceding, and on the least expensive treatment that will yield professionally adequate results.

Major services

The Optional Dental Insurance Plan reimburses 50% of the following eligible services, to a maximum of $1,500 per covered person per calendar year.

  • Crowns, inlays and onlays,

  • Procedures involving gold, only if a lower-cost substitute is not considered consistent with accepted dental practice (otherwise, reimbursement will be based on the lower-cost substitute),

  • Creation and placement of an initial bridge or denture (full or partial), once every 60 months, provided that the appliance was required because at least one natural tooth had to be extracted while the person was covered by the plan,

  • Repair of an existing bridge or denture (at least three years after initial placement; however, the time limit does not apply if repair is required because of an accidental blow to the mouth), and

  • Replacement of an existing bridge or denture if:

    • The existing appliance is at least five years old and cannot be made serviceable,

    • The existing appliance is temporary and is replaced within 12 months of its placement by a permanent one (however, the time limit does not apply to children up to age 18, in the case of accidental injury),

    • The initial appliance was irreparably damaged as the result of an accidental blow to the mouth, or

    • One additional natural tooth must be extracted and the existing appliance cannot be made serviceable.

Orthodontic services

The Optional Dental Insurance Plan reimburses 50% of the following eligible services, to a lifetime maximum of $2,500 per covered person.

Treatment and appliances for the realignment of teeth or jaws using braces or other dental procedures to straighten teeth and keep them in the correct position.

Exclusions

The Optional Dental Insurance Plan does not reimburse expenses for:

  • Cosmetic surgery or treatment, unless it is relating to an accidental injury,

  • Replacement of lost, missing, or stolen appliances.

Apply, claim, modify or end your coverage

To apply for this insurance, to initiate a claim, to modify or end your coverage, login to the Canada Life Secure Site - You will then need the following:

  • your contract number, which is 177714
  • your certificate number, which is your 9 digit uOttawa employee number, and
  • your password. If you do not have a password, you must first sign up online

Waiting Period

If you apply for Optional Dental Insurance coverage more than 31 days after becoming eligible, you will have a waiting period before coverage begins. You do not pay premiums during this waiting period.

Ending your coverage

Canceling is possible only if you have contributed to the plan for a period of at least 2 years.
